As a Hospice Aide at AseraCare Hospice, you know hospice care is a journey, not an end. That's why you'll nurture our patients, providing comprehensive support to make life easier. Partnering with AseraCare Hospices's spiritual care coordinators, social workers and counselors, you'll empower family members and other caregivers to work with patients, enacting a team approach to care on every level. Each day, you will provide personal care, assist with mobility, and maintain a clean and comfortable living environment. Patients' individual care plans will be a constant focus as you set high standards and go to any length to ensure the most peaceful experience for all. We are seeking individuals with a minimum of one year of experience who are independent, accountable, and can think on their feet in an ever-changing environment.
GENERAL PURPOSE
Under the direct supervision of a registered nurse, provides personal care for the patient and assists with activities of daily living. Supports the Provider Relations activities of the organization.
ESSENTIAL JOB DUTIES
+ Follows the interdisciplinary plan of care. Is alert to the patient's needs and concerns. Performs simple procedures as an extension of therapeutic services in accordance with the interdisciplinary plan of care. Attends patient care conferences as requested by the Patient Care Coordinator.
+ Provides personal care, including: bathing, dressing, grooming, caring for hair, nail care, oral hygiene, skin care, and foot care. Assists patients in an out of bed to ambulate, including turning in bed, getting up in a chair, using a walker, wheelchair, or other mechanical equipment when applicable according to the plan of care. Assists with elimination, including: bed pan, urinal, up to the bathroom, routine foley care, and external colostomy and skin care, according to the plan of care. Assists patients with meals, if necessary. May assist with medications that are ordinarily self-administered.
+ Performs household services essential to maintaining a safe and comfortable environment.
+ Observes patients when giving care and reports changes in the patient's condition and needs to the Patient Care Coordinator.
+ Completes appropriate documentation in a timely manner.
+ Participates in ongoing educational programs.
+ Contributes to team effort by accomplishing related results as needed.
CODE OF CONDUCT

Position Requirements
QUALIFICATIONS
+ High school diploma or equivalent preferred.
+ Must be certified by the state as a certified nurse aid or home health aide from an approved training program, as required by state and federal regulations.
+ Successfully completes the agency Home Health Aide Competency Exam.
+ Maintains CPR certification.
+ Must have 24-hour access to a motor vehicle and maintain personal auto liability insurance coverage.
+ Must maintain a valid driver's license.
+ Must meet all local health regulations, pass post-offer drug test and pass post-employment physical exam if required.
+ Must be capable of performing the essential job functions of this job, with or without reasonable accommodation.
+ Must be capable of maintaining regular attendance
KNOWLEDGE, SKILLS, ABILITIES & BEHAVIORS
+ Must be able to read and write English.
+ Must be able to maintain confidentiality regarding patient and company proprietary information.
+ Must have the ability to relate professionally and positively and work cooperatively with other employees at all levels.
+ Must possess computer skills of the types and at a level necessary to accomplish the job.
PHYSICAL AND SENSORY REQUIREMENTS
Medium work involves lifting no more than 50 pounds at a time with frequent lifting or carrying of objects weighing up to 25 pounds. If someone can do medium work, we determine that he or she can also do sedentary and light work.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
